    Nurse's Announcement

    In preparation of re-opening schools, please bring in or mail your student's Current Health Plan, Medicaiton Order, and Vaccination History or Exemptions. 

    If your student is missing any of these important documents they will not be allowed back into school.

  • Daily Student Health Attestation Form

    Please find attached to this link the Student Attestation Form. All Students must provide an Attestation Form to enter our buildings.

    Yellow Attestation Forms will start being sent home. You are welcome to print these forms (on white paper) at home so they can be filled out before sending your student to school or having your student ride one of the school busses. 

    Students who do not present this Attestation Form when entering the building will be unable to attend class until they have been screened by building staff. This may result in delayed arrival to class. If the district screening process results in isolation, parents will be asked to pick the student(s) up from school. 

    If we learn you may have been exposed to COVID-19 during your visit, we will only share this information with public health officials. We will not use this information for any other purpose.
