Please fill out the above Volunteer Clearance Disclosure form so that we may perform a Volunteer Clearance Background Check. Clearance lasts for two years from the initial date of your clearance. Clearance checks are run by the district office and will be passed along to your student's school. Feel free to check with your student's school office or classroom teacher if you are a returning volunteer, before submitting a new form.
