     DAILY STUDENT HEALTH ATTESTATION FORM 
     
    Please find attached to this LINK the Student Attestation Form.  All students must provide an Attestation Form to enter our building.
     
    Yellow Attestation Forms will start being sent home.  You are welcome to print these forms (on white paper) at home so they can be filled out before sending your student to school or having your student ride one of the school busses.
     
    Students who do not present this Attestation Form when entering the building will be unable to attend class until they have been screened by building staff.  This may result in delayed arrival to class.  If the district screening process results in isolation, parents will be asked to pick the student(s) up from school.
     
    If we learn you may have been exposed to COVID-19 during your visit, we will only share this information with public health officials.  We will not use this information for any other purpose.

    YEARBOOK INFORMATION
     
    Get your yearbook today for the 2020-2021 school year.  Only yearbooks paid for by December 16th will be ordered.  Students are working hard to create a yearbook that tells the story of this year.  The class is designing and working on many different stories, collecting pictures and interviews to feature in this year's book.  Please support these students by paying for your yearbook in the office or online.  The cost of the book is $25. 
     
    Yearbooks will only be available for sale through 12/16/2020.
