    Important FOOTBALL Notes:

    ***  LMS 7th- and 8th-grade football seasons begin on TBD ***

    All eligibility paperwork MUST be completed online via Final Forms (available August 1, 2020) in order for players to be eligible to begin practicing.  NO paperwork will be accepted via HARD COPY (other than an athlete's current doctor's physical clearance form); all eligibility paperwork MUST be completed online. 

    To be eligible to physically practice the first day of the season, players MUST be fully registered and cleared with the required paperwork and eligibility forms through the FINAL FORMS website that LMS began using this school year (LHS also uses the program).   Final Forms can be accessed through the main LMS webpage, where directions on how to access the site, along with a "Parent Playbook" about the program are included.

    The only required hardcopy paperwork required to be submitted to the LMS office is a copy of the athlete's doctor's physical (these are good for 2 years).

     ASB and Transportation/Activity fees MUST be paid by the time of the first game to be eligible to play in any games (ASB - $20, Transportation/Activity - $60). 

    Physicals are required of all student-athletes at LMS. They are good for a period of 2 years.  Doctor's physicals may be obtained through a family doctor, or families may choose (this year) to take advantage of a PTN-sponsored physical by local doctors for a $25 donation (donation money will be given back to the school).  Physicals and preliminary fall sports sign-ups will be available on Tuesday, August 20, 2019, from 9am-12:30pm at Lakeside High School or on Tuesday, August 27, 2019, from 3:30pm-6:30pm).  Please print off the forms for the physical (https://drive.google.com/file/d/1dbwBO3wAB3GYF2K9FxedX3hL8nggz_Nq/view) and take with you that evening.  This process is a walk-in process; no preregistration is required.
    Football at Lakeside Middle School is open ONLY to 7th and 8th graders.

    General Information
    Practices on school days will run from 2:30pm-5:00pm, and there may be occasional Saturday practices or games, and the season usually ends around the 25th of October.  There are NO Sunday practices or games (WIAA rule). 
    Players MUST have all eligibility paperwork on file with the school office to be eligible to begin practicing.  All fees ($20 - ASB & $60 Activity Fee) MUST be paid by the time of the first game for a player to participate in game action. 
    Football players MUST participate in 12 practices before being permitted to play in a game.  Games are typically played on Wednesdays (either at 4pm or 6pm), with the exception of one game (occasionally two) of the season that is held on a Saturday. 
    Our teams each play a six-game schedule (maximum allowed by the state association, WIAA - www.wiaa.com).  We strive to have a balanced schedule of home and away games, but that does not always occur.  Some seasons may include a Jamboree prior to the first scheduled official game.  Please check the schedule.
    Most of our scheduled home games are played at LAKESIDE HIGH SCHOOL, but please look at your athlete's schedule or check here for revisions/updates.  Teams return from away-game sites at various times, so please look at your athlete"s schedule for an approximate return time, but those times can change.
