                           ALL NECESSARY FORMS NEED TO BE COMPLETED AND FEES NEED TO PAID BEFORE  THE ATHLETE CAN TURN OUT FOR ANY LAKESIDE HIGH SCHOOL SPORT.

Please turn in your packet to the  main office  no later than 2 school days  prior to the 1st turnout of the sport to ensure eligibility on the first day of practice. 
Failure to meet the deadline may result in ineligibility on 1st tryout.

Full price:     
   First Activity:                   $80.00
   Each Additional Activity:  $60.00
 
Reduced Lunch Price:
   First Activity:                   $60.00
   Each Additional Activity:  $45.00
 
Free Lunch Price:
  First Activity:                   $35.00
  Each Additional Activity:  $25.00
   
Transportation fee is charged for the initial and each additional activity which requires district-funded transportation. This fee must be paid prior to the first competition of the respective season.  In order to qualify for either Reduced or Free status the student must have an approved application on file with the Food Service Department.
