 Middle school athletes planning to participate in any sporting activities during the school year (2008-2009) will need to make sure they obtain a physical and also complete and return all required eligibility paperwork.

Academic Eligibility Requirements

A doctor's physical is REQUIRED before an athlete can begin practicing.  According to WIAA (Washington Interscholastic Activities Association) rules, a physical is now good for 24 monthsAn athlete now only needs a physical every two years!  So that means incoming 8th graders who participated in a sport in 7th grade probably will not need a new one.  Incoming 7th graders, however, WILL need to get one.  Physicals are no longer offered at school, so it would be a good idea to make plans before school begins in September to have a physical performed on your student-athlete.

  In addition to needing a physical, athletes are also required to turn in the following in order to participate in LMS athletics; these may be picked up in the main office beginning about mid-August:                        

  •  ASB Card (Fee)                                  $15.00

  •  Transportation Fee                              $25.00

  •  Code of Conduct/Activity Agreement Form

  •  Insurance Waiver Form

  •  Emergency Treatment Information Card

  •  Sport-specific Safety Guidelines Sheet

All paperwork forms are required for participation; fees must be paid by the first game.
